General follow-up patient appointment instructions

 

• We provide care in Neenah and many of the surrounding areas such as Berlin, Green Bay, Oshkosh, Shawano and Waupaca.
• Typically your provider will order lab testing for you to have done approximately 1 week before your appointment. Please call the office if you are unsure if you need labs done.
• Please arrive up to 15 min prior to your appointment
• Bring your insurance cards to every appointment.
• Your co-pay, if applicable, will be due at the time of the visit.
• Bring your current medication list or pill bottles to every appointment.
• Follow-up appointments are given 30 minute time slots.
• A nurse or MA will take you back to a room where your weight and vitals (blood pressure, pulse, respiration) will be taken. Your Date of Birth, Allergies and current medications and pharmacy will all be verified.
• You will be seen by either a Doctor or Mid-level provider who is under direct supervision of a Doctor.

Billing

The FVNP billing specialist is available by phone Mondays, Wednesdays and Thursdays, from 7am – 3pm. You are always able to leave a message on the confidential voicemail. Your message will be returned within 24 business hours.

NO-Show Policy

• If you have 3 missed appointments with no call prior to cancel we can no longer provide service to you. We are a speciality office, every appointment slot is extremely valuable.
• Please give at least 48 hours notice for any re-schedule needed.
