New Patients
How to become a patient of FVNP
- Referral from your Primary Care Provider. Once our office receives your referral, we will call you to make an appointment at your desired location.
- We provide care in Neenah and many of the surrounding areas such as Berlin, Green Bay, Oshkosh, Shawano and Waupaca. Call our office if you have any questions about establishing care!
- Follow-up care will be arranged upon your discharge when needed after hospitalization.
What to expect
- Please arrive up to 30 min prior to your first appointment.
- Bring your insurance cards which will be reviewed by staff members at reception.
- Your co-pay, if applicable, will be due at the time of the visit.
- Either print, fill out and bring the forms below or be prepared to fill them out when you arrive at our office.
- Be prepared to give a urine sample at the beginning of your visit.
- Bring a current medication list or bring pill bottles.
- New patient appointments are given 60 minute time slots.
- A nurse or MA will take you back to a room where your weight and vitals (blood pressure, pulse, respiration) will be taken. Your Date of Birth, Allergies and current medications and pharmacy will all be verified.
- You will be seen by either a Doctor or Mid-level provider who is under direct supervision of a Doctor.
- Your provider may order you a kidney ultrasound which can either be done at the time of the visit or elsewhere, which is determined on an individual basis.

NO-Show Policy
- If you have 3 missed appointments with no call prior to cancel we can no longer provide service to you. We are a speciality office, every appointment slot is extremely valuable.
- Please give at least 48 hours notice for any re-schedule needed.
