New Patients

How to become a patient of FVNP

Referral from your Primary Care Provider. Once our office receives your referral, we will call you to make an appointment at your desired location.
We provide care in Neenah and many of the surrounding areas such as Berlin, Green Bay, Oshkosh, Shawano and Waupaca. Call our office if you have any questions about establishing care!
Follow-up care will be arranged upon your discharge when needed after hospitalization.

What to expect

Please arrive up to 30 min prior to your first appointment.
Bring your insurance cards which will be reviewed by staff members at reception.
Your co-pay, if applicable, will be due at the time of the visit.
Either print, fill out and bring the forms below or be prepared to fill them out when you arrive at our office.
Be prepared to give a urine sample at the beginning of your visit.
Bring a current medication list or bring pill bottles.
New patient appointments are given 60 minute time slots.
A nurse or MA will take you back to a room where your weight and vitals (blood pressure, pulse, respiration) will be taken. Your Date of Birth, Allergies and current medications and pharmacy will all be verified.
You will be seen by either a Doctor or Mid-level provider who is under direct supervision of a Doctor.
Your provider may order you a kidney ultrasound which can either be done at the time of the visit or elsewhere, which is determined on an individual basis.

If you have 3 missed appointments with no call prior to cancel we can no longer provide service to you. We are a speciality office, every appointment slot is extremely valuable.
Please give at least 48 hours notice for any re-schedule needed.
